Yes, we are accepting new patients of all ages.

Please call (718)-655-7777 | (347)-202-9545 to schedule an appointment or make changes to an existing appointment. 

Please give at least 24 hours of notice for changes or cancellations.

It is recommended that you arrive 15 minutes prior to your appointment in order to allow for time to submit all necessary paperwork and for verification/update of insurance and/or personal information.

Yes, all co-payment and any additional fees are due prior to service.

We accept cash, checks, and debit/credit cards (Visa/ MasterCard/ Discover).

You should bring the following to your first appointment at OFMEDPC:

1. Completed patient forms
2. Current insurance card
3. Photo identification
4. Pharmacy Information (if available)

We also suggest bringing bottles of all current medication. The more information we have on your health and medical history, the better we can serve you.

We do offer same-day appointments. Walk-ins are welcome, but are subject to staff availability and wait time based on severity of illness.
